COPA Community Health Workers

COPA’s Community Health Workers (CHWs) are trusted members of our team who understand the unique needs of our families. They connect patients with essential community resources—from food banks and housing assistance to transportation and job training. This focus on social determinants of health helps address challenges beyond medical care and empowers families to thrive. CHWs screen for these social risks, offer support and guidance and ensure all families have access to the services they deserve, regardless of background.  This commitment to community well-being is what sets COPA apart

How We Help

Who We Help

CHWs help screen any interested COPA family for conditions in their unique environment that may affect a wide range of health and quality-of-life risks and health outcomes. After the screening, CHWs can offer families informal counseling, resources, and information on increasing access to available services in our region, regardless of socioeconomic status.
