ADHD Assessment Process

If you have concerns about your child's attention span, activity level, or impulsivity, COPA is here to help with early intervention. Our ADHD comprehensive evaluation and assessment process begins with a simple phone consultation. When you call our main line at 541.389.6313, ask to be transferred to the ADHD Coordinator. During our initial chat, we’ll answer your questions and guide you through the next steps. Then, we’ll have you complete some forms about your child’s behavior (find those to the right), followed by an in-office appointment with a provider. 

We understand this can be a confusing time, and COPA is committed to providing you with the information and support you need throughout the process.
